The Life Options Patient Scrapbook exists to share photos of patients exercising, playing with grandkids, working—to show that life with kidney failure can be full and active!

We are looking for color photos from all varieties of dialysis patients—younger, older, male, female, every ethnic group, people who are walkers, bike riders, and those more limited in their abilities. Are you a kidney patient with a color photo you love—one that shows you doing something you enjoy? If you have a color snapshot you would like include in our Patient Scrapbook, send it to:

Be sure to include your name, age (optional), type of treatment, and how long you've had kidney failure. Also include a line that says, "I give Life Options permission to use my photo and the information about me I have provided for its website," and sign your name, date, and telephone number. If you prefer, we are happy to use only your first name to identify you. We will mail your photo back if you include your name and address.

If you have a digital photo you would like to share, you can e-mail it to us, in either .jpg or .tiff format. Include in your e-mail the same permission and other information requested for mail-in photos.
